In VPS Lakeshore, Kochi, a Minimally Invasive Aortic Valve Replacement was done on a 57-year-old lady with severe Aortic Stenosis who was presented with dyspnea on exertion and palpitation. Bioprosthetic Aortic valve (Trifecta) was replaced through a 3-inch mini-sternotomy incision under cardiopulmonary bypass. The patient was extubated after an hour postop. And was discharged on 4th post-operative day.Minimally invasive cardiac surgery requires making small incisions to enter the heart through the ribs on the right side of the body, instead of going into the sternum, as is performed on open-heart surgery.Minimally invasive cardiac surgery can be performed to treat several cardiac problems. This form of operation may mean decreased pain andfaster recovery for many patients compared with open-heart surgery. The advantages of Minimally Invasive Cardiac Surgery are:
- Less analgesic requirement
- Less blood transfusion
- Days of hospital stay will come down
- No incidence of VAP (ventilator-associated Pneumonia)
- Early nutrition and ambulation
- Cosmetic outlook.
Factors that might not let a patient undergo MICS:
- Morbid obesity
- Peripheral vascular disease
- Chronic lung pathology
- Emergency CABG
- Malignant arrhythmias
- Diseases of the Ascending Aorta
- Redo Cabg.
Minimally invasive cardiac surgery is exclusively done at VPS Lakeshore Hospital, Kochi for Coronary artery disease ( MIDCABG), valvular diseases - aortic valve replacement, mitral valve replacement/ repair, tricuspid valve repair/replacement, Adult congenital heart diseases - ASD - Atrial septal defect closure, complex surgeries like Mini Bentall procedure.
